Checklist & Tips
to Creating Your Story
1. Brief presentation with the following information saved in PDF format labelled with the MiCD Global Network Membership number (File Size < 10MB)
Note: Patient complaint , diagnosis, case selection, Restorative Plan, Type and shade of SHOFU bioactive composite used, final outcome, before and after images – macro (patient smile) and micro (intra oral view)
2. High resolution before and after images (jpeg format < 2MB) macro (patient smile) and micro (intra oral view) – (please watermark your clinical images ONLY with the MiCD Global Network Membership number)
3. Create a video on the Smile Transformation Story – labelled with the MiCD Global Network Membership number (duration < 2minutes, file size < 100MB)
Note : Patient complaint and expectations, restorative plan and how you met patient expectations, what they liked about the SHOFU bioactive Composites, patient response to the final treatment outcomes
4. Obtain patient consent to share the images and videos with the MiCD Regional Smile Challenge organizers
